The human urethra serves the emptying of the blister. If a narrow place originates in this channel, the urine cannot run off any more unhindered. Beside the narrow places which originate from scar fabric and are called urethral strictures an enlarged prostate gland or tumour can be also responsible for a narrowing. Hence, all together men are concerned substantially more often by an urethra narrowing, than women.
A narrow place in the urethra is caused in most cases by injuries of the mucous membrane, rather more seldom by infections or false educations. This narrow place, therefore, is problematic because the urine cannot run off any more completely what leads to constant urinary way infections and can cause even a back up till the kidneys. In the worst case this can damage the kidneys even permanently. The patient notices first only that the uric ray has become weaker or looks even divided or watering can-like. If the urine cannot be completely emptied any more, a tiny amount remains constantly in the blister and establishes an ideal fertile soil for bacteria. These cause a cystitis which expresses itself by pains with the urination and frequent urge to pass water. If the urine further collects, the infection spreads out to the upper urinary ways, up to the nephritic washbasin. In this case the patient complains about flank pains. A urethral stricture mainly is diagnosed by an experienced urologist. He examines urine and blood to assess whether an urinary way infection or possibly already nephritic damages are given. In addition, there is a row of the technical investigations which can confirm the suspicion on a urethral. On the one hand the pressure of the urinary ray is measured, on the other hand picture-giving procedures, like ultrasound, X-ray examination and in the extreme case a cystoscopy are used to get an impression of the state of the urinary ways.
If the suspicion has been confirmed, a suitable therapy method must be chosen with the help of the findings. Here one makes a distinction between conservative and surgical procedures. The distention, so bougienage of the urethra is the oldest method of treatment with urethral strictures. The implementation is simple and can be carried out even by the patient itself. Unfortunately, the success of treatment is not always given with lasting effect, because the treatment itself can irritate the mucous membrane and
cause injuries which release the education of scar fabric again. In the worst case the narrow place is thereby still expanded.
Another therapeutic approach is keeping open the urethra using a stent. This method is rarely used because the stent is too heavily colonized with germs , causing widespread infections . In addition, the scar tissue grows through the mesh further , making a long-term success is unlikely. Moreover, the scar fabric grows by the meshes further what makes a long-term success unlikely. With the first-time appearance of the urethral stricture a slotting of the urethra can bring success. However, in about 50 percent there is a relapse which cannot be treated then any more successfully by a renewed slotting.
In addition, there is the opportunity of an urethra plastic with which the struck places are removed and the ends are sewn with each other or are joined by transplanted tissue. Though the reoffending rate is very low indeed, the intervention is complicated very much and several hours, a load which is not to be represented for patients with heavy accompanying illnesses last. In the final effect there is under the called procedures no satisfactory therapy method which is suited for most patients. Hence, a new procedure was developed with which mucous membrane cells are taken from the oral mucosa and are bred in the lab so long, until a graft originates in the desired size which can be used instead of the ill urethra mucous membrane.
